TitleSensitivity and specificity of ICGA and BAF in patients with active primary choriocapillaritis.

Purpose To evaluate and compare the sensitivity and specificity of ICGA and BAF in detecting inflammatory activity during follow-up of primary choriocapillaritis
MethodsA cross-sectional study reviewed the records of patients with primary choriocapillaritis who underwent ICGA (Indocyanine Green Angiography) and BAF (Blue Autofluorescence) within a two-week interval (2014–2024) at the Inflammatory Eye Disease Clinic, Asociación para Evitar la Ceguera en México I.A.P. Choriocapillaritis activity was assessed using ICGA and BAF images reviewed by two uveitis specialists. Interobserver agreement and diagnostic performance were analyzed, with ICGA as the gold standard
Results23 eyes from 12 patients (mean age 45±31 years, range 14–62) were included; Females represented 57% (13 eyes).
The most common choriocapillaritis phenotype was idiopathic multifocal choroiditis, affecting (60.87%, n=14 eyes), followed by punctate inner choroiditis (13.04%, n=3 eyes), serpiginous choroiditis (8.7%, n=2 eyes), and other forms of choroiditis (17.39%, n=4 eyes).
Interobserver agreement for ICGA and BAF activity was k=0.62 (p<0.001). Concordance between ICGA and BAF activity was k=0.18 (p=0.64). Clinical examination showed no significant correlation with BAF (p=0.66), while ICGA was significantly correlated (p=0.009). BAF had a sensitivity of 33.3% and a specificity of 47.1% in detecting choriocapillaritis activity
ConclusionICGA and BAF interpretations are consistent among uveitis specialists, but the two are non-interchangeable for detecting inflammatory activity. BAF has low diagnostic performance, with higher specificity than sensitivity. ICGA remains the gold standard for monitoring primary choriocapillaritis
