TitleReal-World Outcomes of Fluocinolone Acetonide Implant for Non-Infectious Uveitis: Experience from a Tertiary Centre in Portugal

PurposeTo evaluate the real-world efficacy and safety of 0.19mg intravitreal FAc implants in preventing relapse in NIU-PS.
MethodsThis retrospective study analyzed 17 eyes from 12 patients treated with FAc implants between 2022 and 2024, with ≥6 months of follow-up. Data included best-corrected visual acuity (BCVA), central retinal thickness (CRT), intraocular pressure (IOP), and inflammation recurrence requiring top off with dexamethasone (DEX) implants. Outcomes were assessed at baseline and up to 30 months post-injection.
ResultsThe study included 17 eyes, 12 of them pseudophakic and one which undergone prior vitrectomy. Etiologies included Sarcoidosis (5 eyes), Birdshot choroidoretinopathy (4), post-surgical macular edema (1), Nivolumab-induced VKH (2) and undifferentiated anterior, posterior and panuveitis (5). At baseline, all eyes presented cystoid macular edema (CME). Mean follow-up was 14.6 ± 7.0 months. By the last visit, inflammatory activity had resolved in all eyes but CME was still present in 3 patients. Compared to baseline, significant improvements were observed in both BCVA (Δ+12.1 ETDRS letters; p<0.01) and CRT (Δ–45.2 µm; p<0.05). During follow-up, 3 eyes experienced disease recurrence requiring DEX implants at 14 months post-FAc, on average. IOP remained stable, with a mean decrease of 0.5 mmHg, and only 1 eye required IOP-lowering drops. No cases required glaucoma surgery.
ConclusionThe FAc implant is a safe and effective option for controlling inflammation in NIU-PS, particularly in patients with uveitic CME. This real-world study demonstrates its potential as a durable, safe alternative to systemic therapies, offering sustained disease control.
