Purpose | To study the clinical pattern, imaging characteristics and outcomes of CNVM associated with Toxoplasma retinochoroiditis (TRC) |
Methods | Retrospective, case series |
Results | CNVM was a rare complication (6%; 15 eyes of 15 cases) in cases TRC (248 cases). Mean age of presentation was young at 19.85±10.12 years with male preponderance (M11; F4). We observed four patterns of presentation: Group A: Active retinitis; early CNVM (within 6 weeks of TRC) Group B: resolving/partially active retinitis; late CNVM (≥6weeks) Group C: healed retinitis, active CNVM Group D: healed retinitis. Predominantly Scarred CNVM. A total of 24 antiVEGFs (Bevacizumab 18, Ranibizumab 6). Mean number of injections/eye needed was 2.18. Group A-C had four cases each while group D had three cases. Of the 12 active CNVMs – classic CNVMs (10 eyes); RAP lesion (2 eyes); All the cases the CNVM was seen adjacent to the TRC lesion. OCT was used to monitor treatment response while OCTA and FFA were required for initial diagnosis. The resulting scarring of the CNVM could be identified as a subretinal fibrotic scar with variable thickening as compared to pigmented, atrophic, thinned and variable excavated scar of TRC. |
Conclusion | CNVM in TRC is a rare association seen more commonly in young adults. CNVM could be noted either during early (6 weeks) or late (>6 weeks) after presentation as active TRC; or later after healing of TRC. The CNVMs resolved faster with a mean 2.18 injection with a subretinal fibrotic scar which could be distinctly differentiated from adjacent healed retinitis lesions. |
