TitleBilateral non granulomatous anterior uveitis and optic disc oedema in a patient with retinitis pigmentosa

PurposeTo report a case of bilateral non granulomatous anterior uveitis and optic disc oedema in a patient with retinitis pigmentosa (RP).
MethodsAn 18-year-old male presented to the emergency department complaining of left eye photophobia, redness and pain for few hours. Past medical history included spasmus nutans. Visual acuity was 5/10 in the right eye and 4/10 in the left eye. Relative afferent pupillary defect was negative and color vision was full in both eyes. Anterior segment examination showed cells +1 in the right eye and +2 in the left eye with normal intraocular pressure. Dilated examination showed typical signs of RP, mild vitritis and indistinct optic disc margins in both eyes. Macula appeared unaffected.
ResultsAutofluorescence and visual fields examinations were typical for RP and fluorescein angiogram showed disc leakage with no signs of peripheral vasculitis in either eye. Ocular ultrasound was unremarkable. Brain MRI and chest X-ray were normal as well and screening for infectious and autoimmune diseases was negative. Blood pressure and body mass index were normal. Neurological and rheumatological consultations did not reveal any pathology although patient refused to undergo lumbar puncture. Due to the optic disc oedema we decided to administer one gram per day of methylprednisolone intravenously for three days and then switch to oral steroids. He was also given topical dexamethasone 0,1% and cyclopentolate 1% drops in both eyes.
On follow-up, visual acuity improved significantly in both eyes as well as the signs of inflammation.
ConclusionUveitis in RP patients is not uncommon. This is another case to support the possible inflammatory pathway in the etiology of RP.
