Visual and Vital Prognosis of Intraocular Lymphoma at Hokkaido University Hospital, Japan

Purpose
Intraocular lymphoma (IOL) is one of the most life-threatening tumors in the field of ophthalmology, which tends to disseminate to various organs and central nervous system (CNS). In recent years, diagnostic methods using vitreous infusion fluids and various treatment options have been tried. The aim of this study was to analyze visual and vital prognosis of IOL in a single institution.
Methods
IOL patients diagnosed and treated at Hokkaido University Hospital between January 1, 2005, and December 31, 2019, were retrospectively reviewed from medical records. Patients who were diagnosed with IOL based on vitreous biopsy or extraocular tissue biopsy were enrolled. This study analyzed the intraocular recurrence rate, visual acuity and the 5-year survival rate.
Results
The cohort included 58 patients, 30 male and 28 female patients, with a mean age of 66.7 years at presentation. The histopathological type was predominantly diffuse large B-cell lymphoma (93%). Thirty patients developed in both eyes during the follow-up period. IOL occurred as the primary vitreoretinal lymphoma (PVRL) in 38 cases, as involvements from the primary CNS lymphoma in 12 cases, and as metastasis from other organs in 8 cases. The intraocular recurrence rate was 23.6%, and the 5-year survival rate was 57.2%. Particularly, the 5-year survival rate of PVRL was 60.8%. The average of final decimal visual acuity before death was 0.579.
Conclusion
The vital prognosis of intraocular lymphoma was not good, while final visual acuity of the patients with IOL was maintained.
