TitleChallenges and Strategies for Long-Term Management of Pediatric Cytomegalovirus Retinitis: A Tertiary Medical Center Perspective

PurposeThis study aims to retrospectively analyze pediatric cytomegalovirus (CMV) retinitis cases, focusing on monitoring, treatment, and long-term treatment outcomes for both congenital and acquired infections.
MethodsWe conducted a retrospective case series review of pediatric CMV retinitis cases in a tertiary medical center. Each case was evaluated for clinical presentation, diagnostic methods, treatment approaches, long-term follow-up, and visual outcomes.
ResultsSeveral cases were included: a congenital CMV infection in an infant and some cases of acquired CMV retinitis in immunocompromised children with underlying conditions such as acute lymphoblastic leukemia and Langerhans cell histiocytosis. Long-term follow-up revealed potential for recurrence, development of complications like retinal detachment and immune recovery uveitis (IRU) like intraocular inflammation, and the need for regular retinal examinations even after negative CMV viremia.
ConclusionThis case series underscores the importance of long-term, multidisciplinary follow-up for patients with pediatric CMV retinitis. Key aspects of management include periodic retinal examinations, monitoring of CMV viral load, vigilance for recurrence and complications, and collaboration between ophthalmologists and pediatricians.
