TitleInfective endophthalmitis, a nightmare and bailout
PurposeThe study described 4 years long summary data on all endophthalmitis cases attended in uveitis clinic of a tertiary referral hospital in Bangladesh.
MethodsAll endophthalmitis cases diagnosed at Bangladesh eye hospital, Malibag brach from January 2019 to February 2023 were retrieved and reviewed .
ResultsEndophthalmitis is one of the most dreadful emergencies which requires immediate intervention to save vision. The most critical factor of developing endophthalmitis is intra ocular colonization by microorganisms. Depending on the infectious agents, bacterial and fungal endophthalmitis are the two important categories. Among all endophthalmitis cases (67cases) attended in our clinic 46 cases(68.65%) were exogenous and 21 cases (31.34%) were endogenous. Acute post-operative endophthalmitis was the most frequent category. 82.1% of all isolates were due to gram-positive bacteria, 9.3% were gram-negative and 8.6% of all cases were due to fungi. The most common bacterial pathogens isolated was staphylococcus epidermidis , followed by Streptococcus pneumoniae and Candida was the most common fungus as well as leading cause of endogenous endophthalmitis.Approximately, two third patients recovered by timely intervention by repeated intra-vitreal antimicrobial injections and one third patients required pars plana vitrectomy , one patient required evisceration. Overall, acute post-operative endophthalmitis showed the best visual outcome , while endogenous endophthalmitis had the poorest visual outcome.
ConclusionPresence of co-morbidities and initial visual acuity were significant risk factors for poor visual prognosis in our settings.
