TitleVariety of manifestations of Posner Schlossman syndrome
PurposePosner-Schlossman syndrome, is characterized by acute, unilateral and recurrent attacks of elevated intraocular pressure associated with mild inflammation of the anterior chamber. The clinical course of the disease tends to exhibit a variable pattern, variable frequency and duration of attacks with variable symptom-free intervals. This disease may resolve spontaneously, however, it can also lead to glaucoma, iatrogenic cataract or corneal decompensation.
MethodsWe present two opposed cases of patients with Posner-Schlossman syndrome, one having presented numerous complications secondary to hypertonia and inflammation; the other having less significant manifestations without sequelae.
ResultsThe first case is that of a 51-year-old patient who presented with hypertonic anterior uveitis in the right eye. The diagnosis of Posner Schlossman syndrome has been made by the clinic since 2021 and confirmed by the presence of anti-Cytomegalovirus IgG in the aqueous humor. He has had two crises until now and quickly resolving with hypotonic and anti-inflammatory treatment.
The other case is that of a 47-year-old patient, followed for recurrent hypertonic uveitis of the right eye since 2012, with approximately 1 episode per month. The diagnosis of Posner-Schlossman syndrome was definitively established following the demonstration of anti-Cytomegalovirus IgG in the aqueous humor. At that time, the patient already presented with moderate glaucomatous neuropathy.
ConclusionThese cases illustrate the wide variety of manifestations of Posner-Schlossman syndrome. Disease duration, number of episodes, the average duration of each episode, and the average interval between episodes were found to be significant risk factors for glaucomatous lesions.
