TitleUltra-Wide-Field Green-Light (532-nm) Autofluorescence Imaging in Syphilitic Uveitis
PurposeTo characterize ultra-wide-field fundus autofluorescence (UWF-AF) abnormalities in syphilitic uveitis (SU).
MethodsMethods: Retrospective review of cases of SU consecutively seen at a uveitis referral center between March 2021 and August 2021. Patients who had UWF-AF and pseudo-color imaging performed were included. All images were reviewed independently by two uveitis specialists.
Results22 eyes of 11 patients were included. All patients were hospitalized and received IV penicillin 4 million UI for 14 days. Age ranged from 23-71 years, with an average of 39 and 8 (88%) were men. Two patients (18.2%) had HIV infection. Five main different patterns of UWF-AF were observed: macular placoid hyperAF (n=16; 72.7%), extramacular placoid hyperAF (n=5; 22.7%), periarteriolar hyperAF (n=2; 9.1%), peripheral confluent/irregular hyperAF foci (n=17; 77.3%), speckled/granular mixed hypo/hyperAF (n=6; 27.3%). There were noticeable disparities between UWF-AF and pseudocolor images, with changes more frequently seen in the former. UWF-AF changed with time, slowly improving after therapy.
ConclusionUWF-FAF changes in syphilitic uveitis are very frequent and present with different patterns. These may be found even in the absence of visible alterations on fundus examination, and slowly improve after treatment. Further studies are expected to investigate these UWF-FAF changes in the long-term, and also their possible prognostic implications.
