Subretinal Hyper-reflective Material in Posterior Uveitis
Purpose
To analyse Subretinal Hyper-reflective Material (SHRM) in Posterior Uveitis using Swept-Source Optical Coherence Tomography (SS-OCT) and SS-OCT Angiography and study the role of anti-VEGF therapy in eyes with vascular SHRM.
Methods
Prospective randomized control study. SS-OCT and SS-OCTA used to study SHRM character and identify choroidal neovascular (CNV) complex within SHRM. Eyes with vascular SHRM randomised to treatment (Group I)/observation(group II) while those with avascular SHRM were observed. Group I received intravitreal Ranibizumab (0.05mg/0.05ml). Change in SHRM dimensions, character and flow from baseline studied in both the groups at 6 month follow up.
Results
Two-third (28/42) of the eyes with SHRM harboured CNV (Vascular SHRM) at presentation. Significant decrease in SHRM height, width and SHRM flow in treatment group was observed.
Conclusion
High reflectivity and increased OCT transmission through SHRM suggest avascular SHRM. Presence of Pitchfork sign and Intra/subretinal fluid indicate presence of intrinsic CNV. All SHRMs with CNV complex do not require treatment but need closer follow up than the eyes with avascular SHRM to look for the signs of activity.
