Grading vitreous inflammation using ultra-wide field imaging in uveitis patients
Purpose
The grading of vitritis is currently based on subjective classifications that compare the degree of clinical inflammation with that of standardized photographs of the posterior pole. At a time of repeated lockdowns, the contribution of teleconsultation using ultra-wide field (UWF) imaging can prove to be major in uveitis patients. UWF imaging allows to assess the degree of vitreous inflammation but also the extreme retinal periphery (foci, vasculitis, etc.). The objective of this work was to study the validity of 2 UWF imaging techniques for the grading of vitritis.
Methods
Prospective study of successive patients with uveitis, seen in the ophthalmology department of Pitié Salpêtrière between June and July 2020. All patients underwent clinical grading of vitritis (Nussenblatt score), followed by an a posteriori evaluation on fundus pictures taken with 2 UWF imaging devices (California, Optos, Clarus, Zeiss).
Results
Fifty-two eyes from 43 patients (21 men) were included. The mean age of patients was 48 +/-19 years. The mean vitritis grade (by slit lamp) was 1.2+ (range 0-4). The vitritis score was 1.51+ (range 0-4) on Optos and 1.42+ (range 0-4) on Clarus. The degree of vitritis as graded on UWF imaging was highly correlated to the clinical score (r = 0.92, p <0.0001 and r = 0.89, p <0.0001 for slit lamp grading versus Optos and Clarus respectively).
Conclusion
UWF imaging can provide reliable grading of vitritis in uveitic patients, regardless of the degree of posterior inflammation. The presence of cataracts and anterior segment inflammation could be a limiting factor to consider. UWF field imaging could be used to remotely assess the degree of posterior inflammation in uveitis patients.
