TitleIncidence and characteristics of endogenous endophthalmitis and chorioretinitis in patients with hematogenous infection
PurposeTo estimate infectious ocular lesions rate and risk factors in patients with bacteremia, funguemia or sepsis admitted to hospitalization.
MethodsRetrospective observational cohort, in hospitalized patients with bacteremia, funguemia or sepsis evaluated by fundoscopy. Associated risk factors, presence of extraocular infectious source, and isolated microorganism were assessed.
ResultsWe included 287 patients. The overall incidence of chorioretinitis was 3,5% (n=10). Only one of them (0,35%) reached a state of endogenous endophthalmitis. Ophthalmic lesion rate in patients with bacterial hematogenous infection was 0.0127%, and fungal chorioretinitis appeared in 0,21% of patients with funguemia. Risk factors found in our cases were: hemodialysis (n=4), recent surgery (n=4), diabetes (n=2), hematologic malignancy (n=2), immunosuppressive treatment (n=2), chronic use of corticosteroids (n=2). Blood culture was positive in 6 patients, urine culture in 1, another patient had soft tissue infection and in 2 patients the source of infection could not be identified. Staphylococcus aureus (20%) and Candida Albicans (20%) were the more prevalent microorganisms. Only one patient (10%) reported symptoms.
ConclusionOcular endogenous infection represents a visual threat in patients with bacteremia and funguemia, this study attempts to identify the frequency and risk factors in our region.
