Sclerocorneal involvement in Behçet's disease: An unusual presentation

To report an uncommon presentation of Behçet's disease which manifested as a sequential and bilateral sclerokeratitis

A 28-year-old male with a past medical history of a deep vein thrombosis in the left lower extremity, his complaint started one year earlier with pain, redness and blurred vision of the left eye (LE). Slit lamp examination revealed the presence of a diffuse scleritis with keratitis and corneal edema, the fundus examination was unremarkable in both eyes. The diagnosis of herpes sclerokeratitis was suspected and the patient was treated with oral antiviral treatment and topical corticosteroids with a good recovery. One year later, he was referred to our department complaining of pain, redness and blurred vision of the right eye (RE). The exam showed a diffuse scleritis with keratitis associated to a diffuse corneal edema, the patient was treated with topical corticosteroid alone. At the follow up we noticed the presence of a bilateral non-granulomatous uveitis with a localized inflammatory vitreous exudate overlying the optic disc with macular exudate. The diagnosis of Behçet's disease was made, immunosuppressive and oral corticosteroid therapy was started with a total remission.
Conclusion
The bilateral and sequential sclerocorneal involvement in Behcet’s disease is unusual and this lack of awareness was responsible for the delay in the diagnosis and the therapeutic management.
