Evaluation of serum angiotensin converting enzyme and lymphopenia in presumed sarcoid uveitis
Purpose
To evaluate the diagnostic efficacy of elevated serum angiotensin converting enzyme (S ACE) and lymphopenia in presumed sarcoid uveitis.
Methods
A retrospective study was conducted on 755 patients with uveitis between January 2019 and June 2020. Demographic, clinical and laboratory data were retrieved. Cases were divided in 2 groups. Group A included presumed sarcoid uveitis and Group B as presumed tubercular uveitis. Measurements of S ACE and lymphocyte count were analysed.
Results
The total 755 patients with mean age (41 ± 13 years) were analysed; 440 (58%) males and 315 (42%) females. Presumed sarcoid uveitis (group A) was diagnosed in 50 (6%) patients, tubercular uveitis (group B) in 222 (29%). The mean age in Group A was 42 + 11 years and in Group B was 40 + 12.9 years. Intermediate and posterior uveitis were common anatomical diagnosis in group A (59% and 20%) and in group B (46% and 38%). High S ACE was 76% in group A and 46.4% in group B (p value <0.05). Lymphopenia was in 20.5% in group A and 22.7% in group B (p value >0.05). High S ACE with lymphopenia was noted in 14% of group A versus 8.5% in group B (p value<0.05). Mantoux test was positive in 20% in group A and 70% in group B (p value <0.05). QuantiFERON TB was positive in 20% in group A and 62% in group B (p value <0.05). High resolution computed tomography was positive in 60% in group A and 89% in group B(p value <0.05).
Conclusion
A combination of elevated serum ACE and lymphopenia is often associated with sarcoid uveitis rather than used alone. Elevated serum ACE is also seen in tubercular uveitis.
