Clinical Characteristics of Endogenous Klebsiella pneumoniae Endophthalmitis: A 13-Year Experience

Purpose
To investigate clinical features and risk factors for poor visual outcomes in patients with endogenous Klebsiella pneumoniae endophthalmitis.
Methods
The medical records of 13 patients (16 eyes) patients with endogenous Klebsiella pneumoniae were retrospectively reviewed between November 2005 and December 2018.
Results
The mean age of patients was 56.23 years, and 10 patients (76.9%) were men. Diabetes was the most commonly associated systemic disease and liver abscess was the major infection source.
Poor initial visual acuity worse than counting fingers was significantly associated with poor final visual outcome (p=0.016). The use of intravitreal dexamethasone injection was not significantly associated with eyeball preservation or good visual outcome in this study. Eyes with initial tap-and-injection showed a tendency to require a secondary pars plana vitrectomy than those with initial pars plana vitrectomy (66.7% vs. 0%, p=0.070).
Conclusion
Endogenous Klebsiella pneumoniae endophthalmitis usually has a poor visual prognosis. Early diagnosis with fair initial visual acuity may be helpful in saving vision.
