Ocular Manifestation of Latent Stage Syphilis in Immunocompetent Patient

Purpose
To report a rare case of ocular syphilis in immunocompetent patient.
Methods
Diagnosis is made based on detailed history taking, complete ophthalmology examination and ancillary test.
Results
A 32-year-old woman came to outpatient clinic of Dr. Saiful Anwar Hospital Malang complaining of blurred vision on right eye since 3 days prior to visit accompanied with eye pain, tearing and headache. She was at 8 months pregnancy. On initial presentation, general examination within normal limit. Her best corrected visual acuity was 6/7,5 on right eye and 6/6 on left eye. There is no sign of anterior chamber inflammation. Dilated fundus examination of right eye showed perivascular sheating and yellowish lesion at superotemporal quadran, while left eye was normal. OCT revealed macular edema of the right eye. VDRL and TPHA test was reactive while other laboratory tests within normal limit. HIV screening test result was non-reactive. She was diagnosed with right eye posterior uveitis related to latent stage syphilis and had been treated with serial injection of penicillin G and oral corticosteroid.
Conclusion
Posterior uveitis is the most frequent manifestation of ocular syphilis in immunocompetence. Other presentation can mimic any eye diseases. Serological tests play an important role in confirming diagnosis.
