Management of Soemmering’s Ring in a case with uveitis associated with juvenile rheumatoid arthritis

Purpose
To report the surgical intervention of Soemmering’s ring in a case with uveitis associated with juvenile rheumatoid arthritis (JRA).
Methods
A 31-year-old female patient presented with decreased vision in her right eye. The patient was diagnosed to have bilateral uveitis associated with JRA at the age of 5 years. Then, she had cataract surgery when she was 10 years old with complicated cataract derived from uveitis in her right eye. Two years later, Nd:YAG laser posterior capsulotomy was performed because of posterior capsule opacification, and five years later, she underwent trabeculectomy surgery for glaucoma in the same eye.
Results
At last admission, her visual acuity was hand motion in right eye and 10/10 in left. Biomicroscopic evaluation yielded widespread anterior synechiae and Soemmering’s ring closing the pupil site in right eye. Therefore surgical intervention was done, anterior synechiae and pupil site was opened. The last visual acuity was 3/10 in her right eye, and no any findings of new uveitis attack in detailed exam. At follow-up period, systemic methotrexate and corticosteroid drug treatment previously given was continued.
Conclusion
The formation of Soemmering’s ring and anterior synechiae in pseudophakic eye with uveitis should be followed carefully. Surgical intervention may be useful in selected cases for improve the visual acuity.
