Infectious Scleritis Masquerading as Immune-Mediated Disease: A Diagnostic Challenge

Purpose
To highlight the diagnostic challenges in infectious scleritis initially diagnosed and treated as immune-mediated scleritis
Methods
Four cases of infectious scleritis treated as immune-mediated scleritis are analysed retrospectively. Clinical presentations, systemic illnesses, treatments received, microbiology from scleral biopsies, and subsequent treatment outcomes were noted.
Results
Four patients, aged 29 to 62 years were included in the study. Each patient presented with unilateral scleritis, initially diagnosed and treated as immune-mediated scleritis with immunosuppressants and oral steroids for durations between three to eight months. All patients underwent scleral biopsy. Microbiology revealed one as a mixed infection with fungal and bacterial, two as bacterial and one as no organisms. Targeted antimicrobial treatment was initiated, resulting in clinical improvement in all cases.
Conclusion
Timely and accurate diagnosis through scleral biopsy should be considered in refractory scleritis.
