TitleIntraocular pressure following sequential intravitreal Dexamethasone and Fluocinolone implant in Uveitis

PurposeTo evaluate changes in intraocular pressure (IOP) following intravitreal fluocinolone implant within 18 weeks of dexamethasone implant in uveitis
MethodsThe study included patients with recurrent uveitis cystoid macular oedema, who received intravitreal dexamethasone implant followed by fluocinolone acetonide implant within 18 weeks of dexamethasone as part of standard care, between Nov 2019 to Sep 2023, at Birmingham & Midland Eye Centre, UK. Variable of interest were changes in IOP post steroid implants & its management.
Results29 eyes received sequential steroid implants. Mean age was 60.8±17.1 years.
Before steroid implants, 5 eyes (17.2%) had glaucoma & had glaucoma laser or surgery & 2 eyes (6.9%) were also on glaucoma drugs.
Mean time interval between the steroid implants was 11.5±4.3 weeks.
After Dexamethasone, high IOP was observed in 8 eyes (27.6%). These cases had no history of glaucoma & were started on topical antiglaucoma medications & 1 case required selective laser trabeculoplasty.
After Fluocinolone, high IOP was documented in 5 eyes (17.2%). 4 of these had high IOP after Dexamethasone & were on topical drugs. IOP settled in 3 eyes with continuation of medications, in 1 case new topical agent was added and in 1 case high IOP was managed by adding anti-glaucoma drug & bleb needling.
Mean IOP before & after sequential steroid implants was 13.2±3.5 & 15.6±5.2 mmHg respectively. Paired t-test was used to compare IOP before & after steroid implants. Mean difference in IOP was 2.4±5.2 and p-value was 0.02
ConclusionSequential implants of dexamethasone & fluocinolone acetonide are safe in majority. Patients with ocular hypertension or glaucoma may have rise in IOP.
