TitleUtility of high frequency Ultrasound Bio microscopy in the surgical decision making of cases presenting with uveitis in a tertiary eye care center.

PurposeTo study the utility of high frequency ultrasound bio microscopy (UBM) in the surgical decision making of cases presenting with uveitis.
MethodsRetrospective analysis between 2007-2023 with minimum 6 months follow-up period.
Results99 eyes of 99 patients underwent UBM for various indications. 66 eyes had opaque media either due to complicated cataract or small pupil with posterior synechae. UBM showed pars plana cyclitic membranes in 59 eyes; as an isolated finding in 19 eyes, in association with ciliary body traction in 20 eyes and with supraciliary effusion in 21 eyes. Normal ciliary body was noted in 4 eyes. Out of the 66 eyes, 26 eyes had visually significant cataract and therefore underwent cataract extraction with intraocular lens implantation with the aid of pupil expanders. Pars plana lensectomy with vitrectomy was performed in 29 eyes to remove the thick pars plana cyclitic membranes. Out of these 29 eyes, 19 eyes had ciliary body traction associated with cyclitic membrane, as visualized on UBM. Among the remaining 11 of 66 eyes with cataract, 5 eyes had visually insignificant cataractous changes with either normal UBM findings or pars plana membranes and supraciliary effusion, hence were medically managed. 2 eyes had gross hypotony with UBM visualising ciliary body atrophy, thus any surgical intervention was deferred. 4 patients refused any treatment. Thus UBM helped in the surgical decision making in 62 eyes out of 99 study eyes and was found to be statistically significant (P- value 0.0060).
ConclusionUBM is an essential tool for surgical decision making in eyes with uveitis and complicated cataract.
