Common Femoral Vein Wall Thickness is a useful diagnostic tool to differentiate Ocular Behçet’s Disease from Other Inflammatory Uveitis
Purpose
Diagnosis of Behçet’s Disease (BD) can be challenging.We showed that common femoral vein (CFV) wall thickness measured by Doppler ultrasonography (US) can be a diagnostic method for the cut-off value ≥0.5mm.It is crucial to make a correct diagnosis since BD's ocular involvement might cause permanent vision loss if left untreated.In this study,we focused on assessing the usefulness of CFV wall thickness measurement in BD-related uveitis.
Methods
42 patients with BD panuveitis and age-gender matched 59 non-BD uveitis patients were included.Demographics,clinical data were recorded during routine visits.CFV wall thickness was measured by radiologist on the same day and after ophtalmologic examination, they were evaluated by a rheumatologist for differential diagnosis.
Results
24 of BD patients and 24 of non-BD patients were male.Among non-BD patients 16 had anterior,7 had intermediate,20 had posterior üveitis.CFV wall thickness of BD patients was significantly higher than CFV wall thickness of non-BD patients(0.77 mm± 0.09 vs 0.49 mm±0.08, p<0.001).CFV wall thickness was ≥0.5mm in 23 non-BD patients.When 23 patients were evaluated by a rheumatologist, oral aphthae in 11 and acneiform lesions in 5 were found.None had genital ulcers or erythema nodosum.Asymptomatic chronic thrombotic changes were detected in 11 patients.BD was diagnosed by expert opinion in 5 patients with oral aphthae, ocular involvement and chronic thrombotic changes.
Conclusion
Our results suggest that measurement of CFV wall thickness by Doppler US can be helpful for the differentiation of ocular BD from other causes of inflammatory uveitis in daily practice.
