TitleHistopathology, immunohistochemistry and molecular biologic study of eviscerated / enucleated specimen of end-stage ocular inflammatory diseases.
PurposeTo describe the histopathology (HPE), immunohistochemistry (IHC) and molecular biologic study of several end stage ocular inflammatory diseases
MethodsHPE, IHC and molecular biologic study of such eyes of 18 eviscerated and enucleated globes secondary to the intractable intraocular inflammation was done.
ResultsAmong cases of tubercular panuveitis, in one eye of subretinal abscess had acid-fast bacilli in the subretinal space. In other three cases, M.TB DNA was detected polymerase chain reaction (PCR). In a case of tuberculous sclerouveitis, HPE showed chronic granulomatous inflammation of the sclera. PCR of the paraffin section was positive for IS6110 gene of M.TB. In a case of end-stage pars planitis, HPE showed acellular fibrous tissue in the peripheral retina with chronic granulomatous inflammation. IHC showed CD3 and CD20 positivity. In a case of Eales’ disease, HPE showed multiple vascular channels surrounded by lymphocytes. IHC showed CD3, CD 8 and CD 68 positivity of the cellular infiltrates. In a case of acute retinal necrosis, chronic non-granulomatous inflammation of the retina with occluded retinal blood vessels was seen. Herpes virus inclusion bodies were seen on HPE. IHC showed CD 3 positivity. PCR was positive for varicella zoster virus DNA. In a case of VKH, predominantly CD 20 positive lymphocytic infiltration in the choroid was seen. In sympathetic ophthalmia, predominantly nongranulomatous inflammation with CD 3 positivity was seen.
ConclusionHistopathology, microbiology, Immunohistochemistry and molecular biologic study can provide valuable insight regarding the diagnosis and pathogenesis of several ocular inflammatory diseases.
