Clinical profile of Viral Necrotizing Retinopathy in a tertiary eye care centre in Eastern India
Purpose
To study the clinical profile of 34 patients with viral retinitis in Eastern India
Methods
Retrospective observational case series between January 2015 and December 2019.Data was compiled based on demography, visual acuity, clinical findings, investigations, treatment and final outcome. Diagnosis was based on the American Uveitis Society Diagnostic Criteria(1994)
Results
41eyes of 34 patients (22males and 12 females) with an average age of 43.96 years (11-70 years) were analyzed. 7 had bilateral involvement. 9 were immunosuppressed. 9 eyes had granulomatous and 18 non - granulomatous anterior uveitis. All eyes had patches of retinal necrosis, vitritis and arteriolar involvement. Polymerase chain reaction detected CMV (7), VZV (8), HSV( 9) and was negative in 10 patients. Treatment included oral antivirals and steroids (20), intravenous injection acyclovir (8), intravitreal ganciclovir (4), injection ganciclovir, oral antivirals and oral steroids (2). 8 eyes underwent prophylactic barrage laser photocoagulation.Complications included optic atrophy (19), retinal detachment (12), cystoid macular oedema (3), vitreous haemorrhage (2), epiretinal membrane(3) and central retinal artery occlusion in 1 eye. Average visual acuity at presentation and final follow up was 0.51 and 1.98 respectively.
Conclusion
Commonest aetiology was HSV with CMV being the commonest amongst the immunocompromised patients. Commonest complication was optic atrophy. Prophylactic barrage laser photocoagulation had a beneficial effect in preventing retinal detachment in our cohort .
