| Title | Could different aqueous humor and plasma cytokine profiles help differentiate between ocular sarcoidosis and ocular tuberculosis? |
| Purpose | To assess cytokine levels in aqueous humor (AH) and plasma of three different uveitis entities: definite ocular sarcoidosis (OS), definite OS associated with QuantiFERON-TB Gold test positivity (OS/Q+) and presumed tubercular uveitis (TBU). |
| Methods | All patients underwent complete eye examination, extensive work-up for uveitis, high-resolution chest computerized tomography and, when needed, mediastinal lymph node biopsy. Diagnostic criteria were used to categorize patients as OS, OS/Q+ or TBU. Blood and AH samples were collected before starting any treatment. |
| Results | Thirty-two patients (15 OS, 5 OS/Q+, 12 TBU) were included. The AH concentration of BLC, IFN-γ, IP-10, and TNF-α was significantly higher in OS than in TBU group. A significantly higher concentration of plasma IL-15, IL-2 and TNF-α was found in OS compared to OS/Q+ group. Both AH IL-8 and MIP-3α and plasma IL-10 resulted significantly increased in OS compared to OS/Q+ and TBU groups. |
| Conclusion | The different AH and plasma cytokine profile observed in OS compared to OS/Q+ and TBU may help to differentiate OS from TBU in overlapping clinical phenotypes of granulomatous uveitis (OS/Q+). |
