The Course of Citomegalovirus Retinitis in Patients with Hematological Disorders

Purpose
To analyze the clinical features and the disease course of cytomegalovirus (CMV) retinitis in immunocompromised patients with hematological disorders.
Methods
Retrospective analysis of the clinical data of 13 patients treated in the hematology service of two referral centers in Israel and the Czech Republic between 2007 and 2017.
Results
Included where 13 patients (11 males) with a mean age at presentation of 40.3 years (median 35, range 14-80). The underlying conditions included multiple myeloma, acute lymphocytic leukemia, acute myeloid leukemia, myelofibrosis, and myelodysplastic syndrome. The mean interval time between diagnosis of CMV retinitis and the underlying hematological condition was 3.35 years (median 2.41 years, range 0.3-12 years).
In all patients, there was a laboratory confirmation of CMV infection by blood and/or ocular polymerase chain reaction. Mean LogMAR visual acuity at presentation was 0.31 and 0.34 at last follow-up. The mean follow-up period was 5 months (median 2.5, range 1-27). Eleven patients passed away in a median period of 14.1 months after being diagnosed with CMV retinitis (median 5, range 1-57).
Conclusion
CMV retinitis is a destructive ocular condition that severely compromises the visual function. It is associated with poor prognosis and increased mortality in compromised patients secondary to hematologic disorders.
