| Title | Comparison of anti-TNF-alpha therapeutic effects in Behçet’s disease and Vogt-Koyanagi-Harada diseas |

| Purpose | Following approval of adalimumab for non-infectious intermediate, posterior, and panuveitis, anti-TNF-alpha treatments have become gradually widely used. However, the efficacies of anti-TNF-alpha for treatment of different etiologies are not well-documented. Here, we compared anti-TNF-alpha therapeutic effects in Behçet’s disease (BD) and Vogt-Koyanagi-Harada disease/sympathetic ophthalmia (VKH/SO).

| Methods | We reviewed the records of patients with BD or VKH/SO treated with anti-TNF-alpha and followed for more than 1 year after treatment at Hiroshima University. Changes in anterior chamber cells, vitreous haze, and active chorioretinal lesion were examined to determine therapeutic effects. Concomitant use of systemic steroid or immunosuppressive agents after anti-TNF-alpha treatment in both groups was also examined. |
| Results | Ten patients with BD (three women) and nine patients with VKH/SO (four women) were included in this study. All VKH/SO patients were treated with adalimumab and five of the 10 patients with BD were treated with infliximab. Ocular inflammation improved in all patients. After starting anti-TNF-alpha therapy, nine of the 10 patients with BD were treated with anti-TNF-alpha therapy alone. However, methotrexate and systemic steroid were concomitantly used in six (66%) and three (33%) patients with VKH/SO. |
| Conclusion | Anti-TNF-alpha therapy was effective for both BD and VKH/SO. Patients with VKH/SO often required a concomitant immunosuppressive agent or systemic steroid.
