The epidemiology of uveitis in a tertiary center in São Paulo, Brazil

Purpose
To describe the clinical profile of uveitis and analyze the changes over a 22-year period in São Paulo, Brazil.
Methods
A total of 691 medical records of uveitis patients attended between March 2020 and August 2023, at a tertiary center in São Paulo, Brazil, were retrospectively reviewed. Age, sex, anatomical and etiological diagnosis were obtained from Redcap database.
Results
Mean age was 47.9 ± 19 years (61.8% female). The age distribution was 0-16 years in 8.5%, 17-40 years in 23%, 41-64 years in 49% and over 65 years in 19.4%. Anterior uveitis, posterior, diffuse and intermediate uveitis were observed in 26%, 25%, 22.6% and 11.7%, respectively. Isolate vasculitis was observed in 8.6% and scleritis in 5% of cases. One-third was infectious uveitis with toxoplasmosis (31.8%), tuberculosis (20.9%) and syphilis (20.4%) as the main causes; cytomegalovirus retinitis (4.7%), acute retinal necrosis (1.9%) and anterior herpetic uveitis (4.7%) were less frequent. One third had non-infectious uveitis with idiopathic juvenile arthritis (16.4%), mainly as anterior uveitis in 0-16 year group, spondyloarthritis (12.9%), Behçet´s disease (13.4%), Vogt-Koyanagi-Harada (12.5%) and sarcoidosis (6.9%) as the main causes. There were 10 cases (1.4%) of primary intraocular lymphoma. One third was idiopathic.
Conclusion
There was an increase of anterior uveitis cases, representing a shift from the study by Gouveia et al., conducted at the same service in 2002, where posterior uveitis was the most frequent (39.7%). Primary intraocular lymphoma was observed only in the present study. Despite advanced diagnostic tests, yet 36.6% of patients remained as idiopathic.
