Vitamin A Deficiency Retinopathy Following Ileocaecal Resection

Purpose
To highlight a case of Vitamin A deficiency retinopathy due to malabsorption and its clinical presentation, diagnostic process, and response to treatment.
Methods
We report a 75-year-old male with a history of ileocaecal resection following stricture and perforation of the ileum caused by large B-cell lymphoma. He presented with a 3-year history of progressive night vision loss and worsening dim-light vision. Extensive investigations revealed low serum vitamin A levels. The patient received five weekly intramuscular vitamin A injections. Electrodiagnostic tests, performed before and after treatment, demonstrated primary dysfunction of rod photoreceptors.
Results
The patient reported rapid and marked improvement in night vision following vitamin A supplementation. He was able to navigate better in the dark and found indoor lighting significantly brighter. Subjective color vision also improved. The patient is now maintained on 6-to-8-weekly intramuscular vitamin A injections.
Conclusion
This case underscores the importance of considering Vitamin A deficiency retinopathy in patients with malabsorption-related conditions and progressive night blindness. Timely diagnosis and treatment can result in significant visual improvement and restoration of quality of life.
