DIAGNOSIS AND MANAGEMENT OF OCULAR ROSACEA IN CHILDREN: A RARE CASE REPORT
Purpose
To report comprehensive diagnosis and management of ocular rosacea in children.
Methods
We report an interventional case report.
Results
A 14-year-old boy presented to the outpatient clinic of Saiful Anwar Hospital Malang with redness, clear watery discharge, photophobia, discomfort and decreased vision that intermittently occurred since one year ago in both eyes. The Dry Eye Questionnaire-5 score was 8. His visual acuity was 0,50 in RE and 0,40 in LE. There are crusts and telangiectasia in the lids. We found bulbar conjunctiva injections and hyperemic also thickening in the tarsal conjunctiva. A pannus was found in the peripheral cornea. Meibomian gland dysfunction expression is grade 2. Schirmer test was 5 mm and meniscus tears were 1 mm. The tear break-up time examination was 3 seconds with a line break pattern. Conjunctiva and corneal spots were not found in the ocular surface staining examination. He has multiple painful papules and erythematous pustules on the center of his face that appeared after exposure to the sun since one year ago. The patient was diagnosed with ocular rosacea with manifestations of RLE limbal stem cell deficiency (LSCD) and dry eye disease (DED). He received fluorometholone, autologous serum, diquafosol natrium, polyvinylpyrrolidone eyedrops, vitamin A eye ointment, and oral omega-3. The patient also consulted to Dermatologist and received oral doxycycline, erythromycin gel, and sunscreen. Four weeks after treatment, the symptoms and signs show improvement.
Conclusion
Ocular rosacea needs a comprehensive examination of ocular surface disease in the lids, conjunctiva, and cornea. A multidisciplinary manner is necessary with the aim of suppressing the inflammatory process.
