Multimodal imaging findings in primary vitreoretinal lymphoma
Purpose
To describe multimodal imaging findings in primary vitreoretinal lymphoma (PVRL).
Methods
The medical records and imaging findings of 5 patients (6 eyes) diagnosed with PVRL, in the department of Ophthalmology Fattouma Bourguiba University Hospital of Monastir, were retrospectively reviewed. Detailed ophthalmic examination, fundus photography, fundus autofluorescence (FAF), fluorescein angiography (FA), and optical coherence tomography (OCT) were analyzed in all patients. Brain magnetic resonance imaging (MRI) was performed in all cases. Diagnosis of PVRL was confirmed by vitreous biopsy in four cases and by stereotactic brain biopsy in one patient.
Results
There were 3 women and 2 male patients with a mean age of 56.4 years (range: 44 and 79 years). Visual blurring was the presenting symptom in all patients (100%). Clinical findings at presentation included vitritis in 6 eyes (100%), vitreous clumps in 2 eyes (33.3%), subretinal deposits in 4 eyes (66.7%), and exudative retinal detachment in one eye (16.7%). FAF showed granular hyperautofluorescence and hypoautofluorescence lesions in 4 eyes (66.7%) and blockage by mass lesion in 1 eye. The most common pattern on FA was hypofluorescent lesions with a "leopard spot" appearance (66.7%). OCT revealed sub-retinal pigment epithelium deposits with an undulating shape (seasick sign) in 5 eyes (83.3%), hyperreflective subretinal lesions in 2 eyes (33.3%), and epiretinal membrane in one eye. MRI of the globes and brain showed brain lesions in one patient.
Conclusion
The diagnosis of PVRL is challenging. Multimodal imaging provides novel insights into features of PVRL.
