TitleTHE LONG-ACTING FLUOCINOLONE INTRAVITREAL IMPLANT EFFECT ON NON-INFECTIOUS UVEITIS: UNICENTER ANALYSIS FROM PORTUGAL
PurposeEvaluate the efficacy and safety of Fluocinolone Acetonide (FAc; 0.2 µg/day) Implant in Non-Infectious Uveitis (NI-U).
MethodsRetrospective observational study where 15 eyes (11 patients) with NI-U treated with FAc Implant in a tertiary hospital were reviewed with a mean follow-up of 19±14 months.
Results93% of eyes presented with macular edema, 33% with choroidal lesions and 13% with vasculitis. All eyes were previously treated with dexamethasone implant, with a mean interval of 6±3 months. At baseline, 40% of eyes were under topical therapy and 67% of patients under systemic treatment. Regarding uveitis activity: 73%, 7% and 20% of eyes presented an A/C cell grade of 0, 0.5+ and 1+, respectively. Likewise, 53%,13%, 27% and 7% of eyes presented a VH grade of 0, 1+, 2+ and 3+. There was a significant greater percentage of patient with grade 0 intraocular inflammation after FAc implant. The median BCVA at baseline was 60±27 ETDRS letters with a median gain of 16 ETDRS letters (p=0,21). CRT at baseline was 382±95 µm with a median reduction of 107 µm (p=0,005). Regarding treatment burden, there was a reduction of 83% (p<0,05) and 10% in the need of topical and systemic treatments from baseline, respectively. Regarding safety, IOP > 21 mmHg was observed in 40% of eyes, 33% needed emergent IOP lowering medication and 13% underwent incisional surgery. All phakic eyes (33%) underwent cataract surgery post-Fac implant.
ConclusionThe FAc implant led to long-term improvements in BCVA, CRT and uveitis activity with a tolerable side-effect profile in this real-world setting fulfilling the need of a long-acting treatment in these patients to minimize chronic uveitis complications.
