TitleRisk factors associated with the progression of cyclitic membranes in pars planitis children
PurposeRisk factors associated with the progression of cyclitic membranes in pars planitis children.
MethodsProspective study of patients with pars planitis, who underwent ultrabiomicroscopic ultrasound (UBM) at diagnosis, 6, 12 months. Demographic, clinical data and fluorescein angiography imaging results were collected. Growth of CM was defined as an increase in longitudinal (anteroposterior in mm) and circumferencial in meridians.
Results86 eyes of 43 patients, 49% girls, ages 7.5±3 years. Initial BCVA 0.9±1 and final BCVA 0.95±1.1. CM was found in 71.4% that increased to 77.9% at 1-year, with longitudinal growth (3.9±2.6mm to 4.4±2.6mm) and circumferential growth (3.3±2.9 to 3.3±2.9).
Basal factors associated with CM were AC inflammation (OR=1.993, 95% CI=1.531-2.795, p<.0001), vitreous inflammation (OR=1.569, 95% CI=1.123-2.283, p<00136), FA activity (OR=2.8, 95% CI=1.207-9.809, p<0.1038), BCVA<20/40 (RR 2.644, 95% CI=1.631-4.933, p<0.0001) and complications (OR=1.398, 95% CI=1.066 -1.843, p<0.0223). Factors associated with CM at 1-year were AC inflammation (OR=1.469, 95% CI=1.108-2.708, p<0.02), BCVA<20/40 (OR= 1.708, 95% CI=1.206-2.790, p<0.0029) and complications (OR=1.554, 95% CI=1.205-2.145, p<0.0021).
With FA activity at 6 and 12 months, 55% had CM longitudinal growth at 1 year; OR=1.818, 95% CI=1.103-4.257, p=.0294 at 6 months and, OR=2.833, 95% CI= 1.338-7.983, p=.0048 at 1-year. And circumferential growth in 40% at 1-year.
ConclusionFactors associated with growth of CM were AC and vitreous inflammation, FA activity, worse BCVA and complications. CM is a biomarker of poor response to treatment or subtreatment so there must be a strict control of inflammation.
