TitleTubercular vasculitis retinae, a case series
Purpose To report a clinical analysis on three cases of tubercular vasculitis retinae along with treatment outcome in a tertiary eye care centre.
Methods An observational case series of three cases of tubercular vasculitis retinae presented at Chittagong Eye Infirmary & Training Complex. Detail history, thorough clinical examination, & Fundus Fluorescein Angiography ( FFA ) were performed. Laboratory test , X ray chest were done . Diagnosis was made clinically.
Results All three cases were young male came for blurred vision. First two cases were siblings, of 24 years and 17 years of age. The third one was 20 yrs of age, a patient of kidney transplantation and pulmonary tuberculosis was on immunosupressive drugs with Anti TB Therapy (ATT).The first two brother had BCVA 6/6. Both of them had +1 vitreous cell,vascular sheathing, retinal haemorrhage and subsequent development of retinal new vessels in both eyes. They had high ESR and positive tuberculine test but normal x-ray chest. They were treated with systemic steroid under ATT and later on Pan Retinal Photocoagulation (PRP) with intravitreal anti VEGF injection. Elder brother suffered from recurrent vitreous haemorrhage and younger one was steroid responder. The BCVA of third patient was 6/9 both eyes, right eye had vitreous cell , peripheral retinal exudates with vascular sheathing. As this patient was already on systemic therapy get only topical treatment.
Conclusion Tubercular vasculitis retinae is a rare disease may develop in same family or in immunosupressive state. Appropriate treatment and meticulous long follow up is needed to take care about complication of disease and also of treatment
