| Title | Corneal tophus associated with gout |
| Purpose | To describe a case of corneal involvement in a patient with a history of gout |
| Methods | A case report |
| Results | A 42-year old male patient with a history of diabetes, vitiligo, and gout presented to our department for a routine examination. The best-corrected visual acuity was 20/20 in both eyes. Slit lamp exam of left eye showed a small paracentral annular corneal lesion. It was superficial, whitish in color, and measuring 0.7mm in diameter with a negative fluorescein-staining test. The rest of the examination of the left eye revealed no abnormalities. Examination of the right eye was unremarkable. Anterior segment optical coherence tomography scan passing through the corneal lesion demonstrated a hyperreflective linear deposit within the epithelium not reaching the Bowman layer and associated with an underlying shadowing. Result of anatomopathological exam of the corneal scraping was consistent with the diagnosis of tophus. |
| Conclusion | The main ocular manifestations of gout usually include vascular changes at the ocular surface in the early stages of the disease. Precipitation of urate crystal has been described in eyelids, tarsal plates, conjunctiva, cornea, sclera, tendons of extraocular muscles, orbit, and lens. Corneal tophus is a rare manifestation, resulting from the precipitation of urate crystal mainly within the epithelium. |
