Could different aqueous humor and plasma cytokine profiles help differentiate between ocular sarcoidosis and ocular tuberculosis?
Purpose
To assess cytokine levels in aqueous humor (AH) and plasma of three different uveitis entities: definite ocular sarcoidosis (OS), definite OS associated with QuantiFERON-TB Gold test positivity (OS/Q+) and presumed tubercular uveitis (TBU).
Methods
All patients underwent complete eye examination, extensive work-up for uveitis, high-resolution chest computerized tomography and, when needed, mediastinal lymph node biopsy. Diagnostic criteria were used to categorize patients as OS, OS/Q+ or TBU. Blood and AH samples were collected before starting any treatment.
Results
Thirty-two patients (15 OS, 5 OS/Q+, 12 TBU) were included. The AH concentration of BLC, IFN-γ, IP-10, and TNF-α was significantly higher in OS than in TBU group. A significantly higher concentration of plasma IL-15, IL-2 and TNF-α was found in OS compared to OS/Q+ group. Both AH IL-8 and MIP-3α and plasma IL-10 resulted significantly increased in OS compared to OS/Q+ and TBU groups.
Conclusion
The different AH and plasma cytokine profile observed in OS compared to OS/Q+ and TBU may help to differentiate OS from TBU in overlapping clinical phenotypes of granulomatous uveitis (OS/Q+).
