| Title | HLA-B27 related exudative retinal detachment in ankylosing spondylitis- A case report |
| Purpose | Report a rare case of HLA-B27 related exudative retinal detachment in ankylosing spondylitis |
| Methods | A retrospective case report |
| Results | A 19-year- old female had ankylosing spondylitis. Her condition was not well controlled with elevated CRP and ESR. She had deterioration of vision in the left eye for 3 days, accompanied with photophobia and eye pain. Her best-corrected visual acuity was 20/20 in the right and 20/500 in the left eye. Fundus exam showed bilateral disc edema and patchy exudative retinal detachment at periphery with retinal folding at posterior pole in left eye. FA/ICGA revealed peripheral retinal vasculitis with fluorescein leakage. Broad laboratory testing was pursued and was positive only for HLA-B27. Vision got improved after 6 doses of pulse therapy with methylprednisolone 500mg, associated with marked decreasing sub-foveal fluid. The prednisolone was tapered over the following months. Her visual acuity of the left eye recovered to 20/20 in 1 month. |
| Conclusion | Although posterior involvement of uveitis associated with ankylosing spondylitis is uncommon; Papillitis, retinal vasculopathy, and even exudative retinal detachment might be seen in poor-control cases. |
