Is optical coherence tomography and fluorescein angiography comparable for diagnosis of macular edema in Behçet uveitis?
Purpose
To assess agreement between spectral domain-optical coherence tomography (SD-OCT) and fluorescein angiography (FA) in diagnosis of macular edema (ME) in patients with Behçet uveitis (BU)
Methods
Medical records of patients with Behçet uveitis who were followed-up in our clinic between January 2016 and August 2021. Of the 67 patients, eighty one eyes that had developed macular edema during follow-up were included in the study. The eyes with any macular leakage (ML) on FA were classified as having ME. Further, ML on FA were graded between 1 for faint perifoveal hyperfluorescence and 4 for evident hyperfluorescence in area greater than 1.5 optic disc diameter. The characteristics of ME by OCT were registered as diffuse/cystoid ME, serous retinal detachment. The frequency and characteristics of discrepant findings between SD-OCT and FA were analyzed.
Results
The median best corrected visual acuity and duration of uveitis were 0.09 (range:1.30) logMAR and 3 (range:33,9) years. In 51.9% of the eyes, ME was observed in both imaging modalities (OCT and FA). OCT showed no ME in 18 eyes. Ten of them had grade 1 ML, 7 had grade 2 ML and one had grade 3 ML on FA. Among 21 eyes without ME on FA, 18 had diffuse ME on OCT. Of the eyes with ME detected by both OCT and FA, 88.1% of them had active inflammation. The proportion of the active eyes was 77.8% in FA positive group and 42.9% in OCT positive group.
Conclusion
OCT and FA showed consistent findings in half of the eyes with ME related to BU. Although OCT seems to be more successful in the detection of ME in inactive eyes, FA and OCT are complementary investigations, that show different features of ME, for the management of patients.
