TitleOcular tuberculosis presenting as conjonctival granuloma

PurposeTo report a case of a female patient with conjonctival granuloma as a manifestation of ocular tuberculosis
MethodsCase report
ResultsAn 18-year-old female patient presented with left eye complaints of tearing and conjunctival growths for 3 months after conjonctival traumatism by a tukey feather.A thorough review of systems was negative.Her best corrected visual acuities were 20/20 od and 20/20 os.On examination, she had vascularized papillomatous-appearing lesions on the temporal and inferior bulbar conjunctiva.No masses were seen on eversion of theupper
lids, and there was no lymphadenopathy in the head and neck region.Anterior and posterior segment examination was unremarkable in both eyes, including a lack of anterior uveitis, iris nodules, vitritis or chorioretinal lesions. Stains for fungi, bacteria, acid-fast bacillus, and spirochetes were negative.Conjunctival cultures were negative.Histologic specimen of conjunctival mass biopsy ,showed caseating neutrophilic granulomas with chronic, compact aggregates of activated macrophages (epithelioid cells) and giant cells.Mantoux test was positive with 17 mm induration diameter. No evidence of pulmonary tuberculosis was seen at chest X Ray. The patient was started Antituberculous Therapy(RIPE).Upon follow-up examination subsequent to treatment, the conjunctival lesions had resolved
ConclusionMycobacterium tuberculosis may lead to formation of conjunctival granuloma .Conjonctival mass biopsy is of utmost importance as it can provide decision making for initiating anti-tuberculosis therapy
