Biological therapy in Juvenile Idiopathic Arthritis(JIA)-associated uveitis cataract surgery

Purpose
Evaluate biological treatment in children affected by JIA-associated chronic anterior uveitis undergoing cataract surgery
Methods
Retrospective study of children affected by JIA-associated uveitis that underwent cataract surgery. Surgery consisted in cataract phacoemulsification with primary posterior chamber foldable hydrophobic acrylic IOL implantation. First group was preoperatively treated with immunosuppressive drugs and/or systemic corticosteroids. Second group was treated with immunosuppressive agents and tumor necrosis factor alpha (TNFα) antagonists (Adalimumab or Infliximab). Main outcome was evaluated by change in the best corrected visual acuity (BCVA)
Results
33 eyes of 26 children were included: 15 eyes (12 children) in first group (mean age of 8.13 years) and 18 eyes (14 patients) in second group (mean age of 10.7 years). Mean time between uveitis onset and cataract surgery resulted longer in 2°group (78.2 months) than in 1°group (36.0 months). Patients treated with biological drugs significantly improved average BCVA from 1.37±0.78 logMAR preoperatively to 0.34±0.26 logMAR at last follow-up(p<0.01). Whereas, in the other group it raised from 1.0±0.75 logMAR preoperatively to 0.98±1.04 logMAR at last follow up(p=0.66)
Conclusion
Preoperative treatment with immunosuppressive and biological drugs allows to perform cataract surgery at an older age and increase visual outcome in JIA-associated uveitis
