Evidence of choriocapillaris flow reduction on swept-source OCT angiography in two typical cases of Multiple Evanescent White Dot Syndrome

Purpose
Our purpose was to report two typical multiple evanescent white dot syndrome (MEWDS) cases in which swept-source OCT angiography (SS OCTA) revealed transient areas of loss of signal at the level of the choriocapillaris.
Methods
The SS OCTA images of two female patients with typical MEWDS were analyzed at the onset of the disease and 6 months later.
Results
The patients were aged of 24 and 25 year-old respectively. Previous medical history was unremarkable in both cases. The diagnosis of MEWDS was made on the basis of clinical findings and typical results of fundus autofluorescence, optical coherence tomography, and fluorescein angiography. At presentation, SS OCTA showed a few small areas of loss of signal at the level of the choriocapillaris with corresponding flow defects in B scan SS OCT on both cases. There was no evidence of signal attenuation related to overlying inflammatory tissue to explain the reduced choriocapillaris flow. At 6 months follow-up, SS OCTA showed complete resolution of choriocapillaris flow-voids, with restoration of flow signals on B scan OCT on both eyes.
Conclusion
Both our patients with typical acute MEWDS showed on SS OCTA multifocal small areas of flow reduction at the level of the choriocapillaris, with full recovery 6 months after initial presentation. This finding supports the hypothesis of transient, primary or secondary choriocapillaris hypoperfusion.
