| Purpose | In this report, a case of uveitic masquerade syndrome which was caused by previous retinal vasculopathy grade C proliferative vitreoretinopathy will be shown |

| Results | A 20 year-old female suffered from persistent severe pan-uveitis in her left eye. Her visual acuity was 6/6.7 in right eye but hand movement in the left eye. Yellowish large cells floating in anterior chamber and deposited on surface of intraocular lens and endothelium of cornea were noted. Iris rubeosis was also observed. There was grade 4 vitreous haze, with retinal detachment and retinal vascularization. Tracing back her history, ocular sarcoidosis has been diagnosed and systemic immunomodulatory therapy have been prescribed. Photocoagulation, vitrectomy and lens extraction have been performed for her retinal condition. The patient lost for follow-up for few years without taking any medications and came back with the worsening of visual acuity in recent months. The uveitis didn’t improved with the treatment of systemic and regional injection of corticosteroid. After re-evaluation of her ocular condition in both eyes, her diagnosis was modified as uveitic masquerade syndrome caused by grade C PVR and chronic retinal detachment in pre-existing retinal vasculopathy. Systemic medications were discontinued and surgical treatment was performed. |
| Conclusion | Chronic retinal detachment is one of the differential diagnoses for uveitis masquerade syndrome. Clinicians should keep in mind that retinal vasculopathy with PVR could present as severe uveitis in chronic status. |
