Short-term outcomes of Tenon's capsule retractor-assisted trabeculectomy for uveitic glaucoma

Purpose
To evaluate the short-term outcomes of trabeculectomy assisted by a novel device, Tenon’s capsule retractor (TCR), for uveitic glaucoma.
Methods
We retrospectively reviewed medical records of 18 consecutive eyes with uveitic glaucoma who had undergone TCR-assisted trabeculectomy. During this surgery, a TCR was used to insert mitomycin C-soaked sponges into the deeply posterior sub-Tenon’s space, a posterior edge of a scleral flap was not secured, and a 3 mm x 6 mm blocking suture was placed over the limbal extreme of conjunctival incision. The outcomes assessed were postoperative changes in intraocular pressure (IOP) and the number of anti-glaucoma eye drops and complications.
Results
The mean IOP significantly decreased from 36.1±9.9 mmHg preoperatively to 10.6±4.2 mmHg at 1 month, and 14.8±5.1 mmHg at 12 months (P<0.001). The mean number of anti-glaucoma eye drops also significantly decreased from 3.7±1.6 preoperatively to 0.2±0.5 at 1 month, and 0.9±1.2 at 12 months postoperatively (P<0.001). Postoperative complications included choroidal detachment (n=6), hyphema (n=3), fibrin deposition (n=3), shallow anterior chamber (n=2), and bleb leak (n=1) during the first 4 weeks, and pupillary membrane formation (n=1), transient hypotony (n=1), and posterior synechiae formation (n=1) thereafter.
Conclusion
TCR-assisted trabeculectomy for uveitic glaucoma was well tolerated and demonstrated a good short-term IOP control.
