Ultrasound biomicroscopy as a tool in the management of uveitic complicated cataracts

Purpose
To study the utility of Ultrasound biomicroscopy in the management of uveitic complicated cataracts.
Methods
Retrospective analysis of 9 eyes of 9 patients with uveitic complicated cataract.
Results
Our series had 9 patients of which 8 were young men. Median age of presentation was 28 years. 6 patients had bilateral disease, presented with sudden diminution of vision with complicated total cataract as presenting feature in one eye along with 3 other patients who had unilateral involvement. All 9 eyes had shallow anterior chamber with peripheral anterior synechae and slit angle. 5 patients presented with hypotony and one patient with ocular hypertension. All 9 eyes underwent B scan and ultrasound biomicroscopy to anatomically localize the inflammation. 6 eyes had ciliary body inflammation, 4 eyes had supraciliary effusion and 8 eyes had parsplana membranes of which 7 caused traction on ciliary body. All patients were started on oral corticosteroids and methotrexate. Surgical decision of doing lensectomy and vitrectomy was taken based on clinical and UBM findings. The mean follow-up period was 3 years. 7 eyes had excellent improvement in vision with stable intraocular pressure at final follow up. 2 eyes had scarred cornea.
Conclusion
Routine phacoemulsification with in the bag intraocular lens implantation is difficult in these situations and cannot address the underlying pathology. Investigation with UBM to look for the exact pathology and anatomical localization of inflammation with subsequent lensectomy and vitrectomy can save the vision.
