Subtenon Triamcinolone Injection in Bilateral Diffuse Uveal Melanocytic Proliferation Syndrome

Purpose
Bilateral diffuse uveal melanocytic proliferation syndrome (BDUMP) is a very rare paraneoplastic intraocular syndrome with limited reported cases. This study aims to present a case report of BDUMP and evaluate the therapeutic effect of subtenon triamcinolone injection.
Methods
A case report.
Results
An 83-year-old male undergoing chemotherapy and radiotherapy for non-small cell lung cancer presented with bilateral visual deterioration. His initial uncorrected visual acuity was 0.15 in the right eye and 0.2 in the left eye. Fundoscopic examination revealed diffuse pigmented changes resembling giraffe spots and exudative retinal detachment in both eyes. Autofluorescence imaging demonstrated hypoautofluorescent lesions surrounded by hyperautofluorescent areas, while optical coherence tomography (OCT) revealed bilateral choroidal folds and outer retinal disruption. Systemic examination also revealed skin pigmentation. Based on these findings, the patient was diagnosed with BDUMP associated with lung cancer. Subtenon triamcinolone injections were administered bilaterally, leading to a reduction in subretinal fluid.
Conclusion
BDUMP is associated with poor visual prognosis and high mortality due to its underlying malignancy. Clinical recognition of BDUMP may help diagnosis of underlying malignancy, and localized steroid treatment, such as subtenon triamcinolone injections, may provide therapeutic benefits.
