Use of Prostaglandin Analogues in the treatment of ocular hypertensive uveitis:

Purpose
Prostaglandin analogs (PGA) have evidence of lowering intraocular pressure (IOP) in patients with glaucoma, but it is controversial to use PGA in ocular hypertensive patients with uveitis due to the increased risk of ocular inflammation, cystoid macular edema, and reactivation of herpes simplex keratitis. We report using PGA in cases with refractory ocular hypertensive uveitis treated with maximal medical therapy (MMT).
Methods
This study included uveitis cases with non-surgical and refractory ocular hypertension under MMT. This pursuit of optimal IOP control, where we explored the efficacy of utilizing three PGA including latanoprost, travoprost, and bimatoprost. IOP data, inflammation status, and adverse events were recorded.
Results
We selected 16 uveitic patients with elevated IOP under MMT, and prescribed PGA for IOP reduction. Lantanoprost is the most well-tolerated PGA agent in these cases. Different PGA seems to have personalized individualization. The possible occurrence of side effects, such as CME, and HSK, was not observed. Most adverse events were found with the use of bimatoprost.
Conclusion
Controlling IOP in non-surgical refractory ocular hypertensive uveitis patients with PGA is challenging, and it is working in selected cases. It is not so frequent to increase the risk of rare complications related to the blood-aqueous barrier disruption
