TitleSuccessful Intravenous Steroid Therapy in Exacerbated Sympathetic Ophthalmia
PurposeTo report a successful intravenous steroid therapy in exacerbated sympathetic ophthalmia case in order to achieve rapid visual acuity improvement
MethodsA case report
ResultsA-39-year-old man came to the outpatient clinic at RSUD dr. Saiful Anwar with complaint of sudden blurring in the left eye. The patient has been diagnosed with sympathetic ophthalmia in the left eye and phthisis bulbi in the right eye since two years ago. Patient had received high dose oral prednisone on tapered dose and cyclosporine 50mg twice a day. Ophthalmological examination in the left eye showed visual acuity (VA) was 1m CF, while one week earlier was 6/24. Anterior segment examination revealed 2+ flares, 3+ cells, and 2+ vitreous cells. Posterior segment examination in the left eye revealed moderate vitritis, vascular sheathing on superotemporal area and Dalen-fuchs nodules on the peripheral retina. The dosage of oral prednisone was increased to 20mg once a day. One week later the VA became 2m CF. Anterior segment examination revealed 1+ flare, 1+ cells and 2+ vitreous cells but no significant improvement in posterior segment examination. The patient was hospitalized and received intravenous methylprednisolone (IVMP) 1 gram per day for three consecutive days. Three days after IVMP, VA improved to 6/30. Anterior segment examination revealed no flare or cells. Posterior segment examination revealed reduced of vitritis. One week after IVMP, VA became 6/18. Eight week after IVMP, VA became 6/15 and no signs of active intraocular inflammation
ConclusionSympathetic ophthalmia can be exacerbated and has progressive course which can result in poor vision. Aggressive management with IVMP is needed to get a better visual and clinical outcome
