Toxoplasmosis chorioretinitis mimicking cytomegalovirus retinitis in an Acquired Immunodeficiency Syndrome (AIDS) patient
Purpose
To report an atypical presentation of ocular toxoplasmosis simulating cytomegalovirus retinitis.
Methods
A case report.
Results
A 48-year-old female with acquired immunodeficiency syndrome presented with decrease vision of her left eye for 2 weeks. Visual acuity was 6/6 in her right eye and 6/15 in her left eye. The slit lamp examination revealed no inflammatory reaction in both the anterior chamber and the vitreous of her left eye. The fundus exam revealed densely yellow-whitish retinal necrosis over temporal lower arcade of her left eye. She was originally diagnosed as CMV retinitis and was treated with valganciclovir. The lesion got improved about 2 months later but recurred about 1 year later. AC fluid PCR was checked and showed negative for viral etiologies including CMV. The serology test demonstrated positive IgG toxoplasma antibody. Under the presumed diagnosis of toxoplasmosis retinochoroiditis, antiparasitic therapy was prescribed. Her retinal infiltrates decreased markedly and symptoms got improved.
Conclusion
Toxoplasmosis infections in in immunocompromised patients can present as necrotizing retinochoroiditis that mimics viral retinitis. Accurate differentiation between this condition and CMV, with early intervention and appropriate treatment may be critical.
