Interesting case of Wilson’s disease with unilateral anterior uveitis

To report a case of unilateral anterior uveitis in a patient with Wilson’s disease

A 50 year old Indian male patient presented with complaints of gradually progressive blurring of vision in left eye (OS) since 2 weeks associated with worsening of pain and redness. Ophthalmological examination revealed a normal anterior segment in the right eye (OD). In the left eye (OS) there was presence of circumcorneal congestion, an isolated keratic precipitate, Kayser Fleischer ring, anterior chamber reaction, along with iris pigments over the anterior lens capsule. Posterior segment examination of both eyes (OU) was normal. AS-OCT of the left eye (OS) further aided in the identification of KF ring. The anterior uveitis was then controlled using topical steroid therapy. Detailed physician evaluation along with relevant investigations led to the diagnosis of Wilson’s disease and Penicillamine therapy was started for the patient. Local steroid was helpful in minimizing the anterior uveitis in the left eye.
Conclusion
Early diagnosis and treatment of Wilson’s disease is helpful in preventing the morbidity and mortality. Uveitis with Wilsons’s disease is a rare occurrence.
