Retinitis with focal superficial retinal deposit as the initial manifestation of syphilis co-infected with HIV

To report a case revealed retinitis with focal superficial retinal deposit secondary to syphilis co-infected with HIV

Results
A 36-year-old Taiwan Chinese male refer to our OPD due to blurred vision and floaters for 2 months. On examination, best-corrected visual acuity (BCVA) was 6/60 on the right eye and 6/30 on the left eye. Slit lamp examination showed mild cells (+) Fundus examination revealed multiple small white dots overlying focal retinitis over superior mid-equatorial retina with marked vitritis. Laboratory examination show positive finding of rapid plasma regain and Treponema pallidum hemagglutination (TPHA). Under the impression of ocular syphilis, enzyme-linked immunosorbent assay and Western blot test were performed and revealed concurrent HIV infection. The patient was treated with intravenous penicillin. In addition, HARRT was prescribed. Rapid resolution of the vitritis and retinal precipitation were noted.
Conclusion
Retinitis with focal superficial retinal deposit as the initial manifestation of syphilis co-infected with HIV. The patients with panuveitis of unknown cause should undergo VDRL and FTA-ABS screening. Besides, subsequent testing for HIV infection in ocular uveitis is necessary.
