Orbital Infarction Syndrome following Functional Endoscopic Sinus Surgery
Purpose
Functional endoscopic sinus surgery (FESS) is a type of surgery performed for paranasal sinus disorders to remove obstructing tissue and allow drainage in the osteomeatal complex. Minor and major ophthalmic complications can arise from this procedure.
Methods
We present the case of an eighteen-year-old male who underwent FESS on both nasal cavities due to chronic rhinosinusitis and subsequently developed orbital infarction syndrome in one eye.
Results
FESS was uneventful and did not go beyond the middle turbinate and maxillary sinus. The ethmoid bulla was not violated but pure epinephrine nasal pack on the left nasal cavity was utilized. Twelve hours post FESS, the patient was referred to the general clinic for unilateral vision loss of the left eye. Progressive periorbital swelling was noted with conjunctival chemosis, complete ophthalmoplegia, and anterior chamber inflammation with increased intraocular pressure . Ocular ultrasound revealed choroidal effusion with vitreous hemorrhage. Orbital infarction syndrome was suspected; steroid therapy combined with sublingual nitroglycerin was the mainstay management. Improvement of clinical signs was observed but vision did not recover.
Conclusion
Orbital infarction syndrome may occur post FESS. It should be suspected when there is acute vision loss after surgery.
